Medication comparison
Semaglutide vs Tirzepatide: an evidence-based comparison.
Two of the most discussed GLP-1 therapies for weight management. Here's how they actually differ — in mechanism, efficacy, side effects and clinical fit for Indian patients.
GLP-1 receptor agonist
Semaglutide
- Single mechanism: activates GLP-1 receptors
- Brands: Ozempic (metabolic), Wegovy (weight), Rybelsus (oral)
- Weekly injection (or daily oral for Rybelsus)
- ~15% average weight loss at 2.4 mg over 68 weeks (STEP trials)
Dual GIP + GLP-1 agonist
Tirzepatide
- Dual mechanism: activates GIP and GLP-1 receptors
- Brands: Mounjaro, Zepbound (US)
- Weekly injection only
- ~20–22% average weight loss at 15 mg over 72 weeks (SURMOUNT-1)
Head-to-head comparison
Based on published trial data (STEP, SURMOUNT, SURPASS) and product labels. Individual results vary.
| Attribute | Semaglutide | Tirzepatide |
|---|---|---|
| Mechanism of action | GLP-1 receptor agonist | Dual GIP + GLP-1 receptor agonist |
| Administration | Once-weekly subcutaneous injection | Once-weekly subcutaneous injection |
| Typical maintenance dose | 2.4 mg (Wegovy) / 1–2 mg (Ozempic) | 5 mg, 10 mg or 15 mg |
| Average weight loss (top dose) | ~15% over ~68 weeks | ~20–22% over ~72 weeks |
| Common side effects | Nausea, reflux, constipation, fatigue (early titration) | Nausea, diarrhoea, constipation, reduced appetite |
| Appetite & cravings | Strong reduction in appetite and food noise | Often stronger appetite suppression in early users |
| Availability in India | Rybelsus widely available; Ozempic limited; Wegovy newly launched | Mounjaro launched in India in 2025; access expanding |
| Approximate monthly cost (India) | ₹4,000 – ₹25,000 depending on brand/dose | ₹14,000 – ₹40,000 depending on dose |
One hormone vs two
Semaglutide mimics the gut hormone GLP-1, slowing gastric emptying and reducing appetite. Tirzepatide does that and activates GIP — a second incretin hormone involved in insulin and fat metabolism. The dual action appears to drive larger average weight loss in trials.
What the trials show
In STEP-1, semaglutide 2.4 mg produced ~14.9% body-weight reduction over 68 weeks. In SURMOUNT-1, tirzepatide 15 mg produced ~20.9% reduction over 72 weeks. Both are substantially greater than placebo or lifestyle alone — and the gap between them narrows with strong adherence and habit support.
Choosing between them
The right option depends on BMI, comorbidities (PCOS, prediabetes, fatty liver), prior medication history, budget, tolerability, and what is reliably available in India. A clinician should make the call — not a search result.
What's the same
Both require slow dose escalation, both can cause GI side effects, and both work best alongside protein-forward nutrition, resistance training and sleep. Neither is a shortcut — stopping abruptly without lifestyle scaffolding usually leads to rebound.
Context for Indian patients
- Both medicines should only be used after a clinician-led eligibility review — BMI, metabolic markers and contraindications matter.
- Cost and supply differ meaningfully in India; the cheapest option is not always the right one for you.
- Self-sourcing from grey-market pen suppliers is unsafe — cold chain, dosing and counterfeit risk are real concerns.
- Long-term outcomes depend more on habit systems (protein, resistance training, sleep) than the molecule chosen.
